TranXition Program description

With the help of a multidisciplinary team, the TranXition Project aims to support youth aged 15 to 25 with disabilities as well as their families in their transition into adult life, with a focus on maximizing their social participation. Though rooted in group intervention, the TranXition Project can also support participants through individual or community intervention.

By identifying goals specific to each participant, the program challenges them to surpass themselves. They push the boundaries of their comfort zone through a range of personal, social, cultural, school, professional, economic and political “real life” experiences. These activities are carried out in the community in collaboration with community stakeholders. By empowering participants and holding them accountable, the TranXition Project strives to create agents of change.

The group intervention culminates in a final experience organized by participants. This experience can take the form of a trip or an outdoor stay.


Inclusion criteria

  • Be between 15 and 25 years of age;
  • Be able to communicate autonomously in English or French (with assistive technologies, strategies or ASL/LSQ interpreter if necessary);
  • Be living with a:
    • Motor Impairment and/or
    • Auditory Impairment and/or
    • Language Impairment and/or
    • Visual Impairment and/or
    • Mild Intellectual Disability and/or
    • Mild Autism Spectrum Disorder
  • Know their medical diagnoses;
  • Know their medication/treatments;
  • Know if they are under protective supervision;

Participants must also demonstrate their autonomy in the following areas:

  • Be able to autonomously reach the street corner, with or without assistive technology (approx. 250 metres);
  • Have reached secondary 2 or higher level of education;
  • Be able to eat independently or direct their feeding;
  • Be able to eliminate waste autonomously.

Services offered

The TranXition Program primarily focuses on group intervention. 

The TranXition Program staff’s primary objective is to ensure the wellbeing and safety of participants and help them surpass themselves. The staff is also committed to working in a multidisciplinary manner to promote a stimulating and creative environment conducive to achieving the participants’ objectives.

TranXition Project objectives

The TranXition Project is structured around four building blocks that represent its objectives:

  1.  (Social isolation) Participants will be given tools to create and maintain social connections.
  2. (Mental health/wellbeing) Participants will learn to identify the things that improve their wellbeing.
  3. (Work/studies) Participants will be able to identify and implement an occupational life plan that aligns with their interests and abilities.
  4. (Residential autonomy) Participants will increase their level of autonomy by identifying their needs and developing their abilities.

Parents will also be equipped to support their child in their social participation.

  1. (Knowledge) Parents will have access to information about resources to help with their child’s transition into adult life.
  2. (Peer or individual support) Parents will have access to support in their child’s transition to adult life.

  • At the Mackay site
  • In the community (most interventions take place outside the rehabilitation centre)

User journey

To access services

  • To be eligible for services, a person must have a specific diagnosis related to physical impairment and report persistent and significant difficulties in carrying out their life habits. The person must also face challenges in their transition into adult life.
  • The referral source must include a copy of the medical report documenting the diagnosis along with all other relevant medical or professional reports (occupational therapy, physiotherapy, psychology, speech therapy) and the Request for Services Form (DSIE Form for professionals working within the Health and Social Services Network).
  • All documents must be sent to Admissions Service de l’accueil, évaluation, orientation (AEO)
