Day Centre

Clientele

  • Persons of 65 years of age or more that meets the following conditions:
    • diagnosed with a vision or a dual sensorial loss (hearing and vision)
    • live on the Island of Montreal
    • request support to maintain autonomy in the community
    • wish to break social isolation

Objective

  • To assist client with a visual or dual sensorial impairment to remain at home, thereby delaying or avoiding institutionalization and fostering an optimal quality of life in the community

Services offered

  • Joint Nursing and Special Care Counseling assessment of client’s health and autonomy in the community with weekly follow ups
  • Psychosocial support for coping with vision loss
  • Support in the practice of using visual aids, assistive listening devices and other technical aids for persons with sensorial loss
  • Support in implementing efficient communication strategies
  • Group activities related to:
    • physical and cognitive health
    • physical fitness and wellness
    • infection control
    • fall prevention
    • safety in the home
    • rehabilitation follow-up
    • activities of daily living
    • digital reader training
  • When indicated, referral to the 25+ Sensorial Impairment Program, other rehabilitation services and CLSC

All services are provided by a team of professionals from the nursing, patient care assistance and special care counseling disciplines.

To access services

  • To be eligible for services, the person must have:
    • a specific diagnosis related to vision impairment with or without hearing loss
    • report persistent and significant difficulties in executing life habits as a result of the vision impairment
  • The referring source must include:
    • a copy of a recent eye report (<1 year)
    • a copy of the most recent audiogram for those with a hearing loss
    • any other pertinent medical and professional reports (Occupational Therapy, Physiotherapy, Psychology, Optometry, Social Work, etc.)
    • Request for Services form or the Demande de services interétablissements (DSIE) for professionals working within the Health and Social Services Network
    • all documents must be sent to the ID-ASD-PD Access Desk

To make a request for services

  • Referral using the Request for Services form can be made by the individual, the family or a member of the community
