25 years + Sensorial Impairment Program

Clientele

  • Any person 25 years of age or more living with one of the following conditions:
    • hearing loss
    • vision loss
    • dual sensorial loss (hearing and vision) or Deafblind
    • tinnitus
    • cochlear implant
    • Deaf who communicate in American Sign Language (ASL)

Services offered

Hearing Impairment, Deaf, Tinnitus

  • Assessment of auditory and communication skills
  • Counselling and training on the use of assistive listening devices and other technical aids for the Deaf and hard-of-hearing persons
  • Counselling and training on efficient communication strategies
  • Counselling and training on tinnitus coping strategies
  • Psychosocial support for the client and significant others
  • Support groups for persons with hearing loss and tinnitus
  • Promotion and support for integration into the community, school, workplace and leisure activities
  • American Sign Language (ASL) classes
  • Access to Deaf Adult Workshop Series (DAWS) activities to acquire personal autonomy and knowledge on current resources

Cochlear Implant

  • Information, assessment and counseling on eligibility for a cochlear implant
  • Access to an intensive functional rehabilitation program following the surgically insertion of a cochlear implant
  • Technical and programming support for cochlear implants

Vision Impairment

  • Assessment and training in learning how to use residual vision
  • Assessment and training for autonomy in activities of daily living
  • Assessment and training in orientation and mobility
  • Assessment and training in communication skills
  • Assessment and training in the use of adapted technology
  • Psychosocial support and counseling for the client and significant other
  • Promotion and support for integration into the community, school, workplace and leisure activities

Dual Sensorial Impairment and Deafblind

  • Access to all the above services

All services are provided in collaboration with the client and his significant others by a multidisciplinary team of professionals from the following disciplines:

  • Orientation and Mobility
  • Psychology
  • Social Work
  • Special Education
  • Speech and Language
  • Vision Rehabilitation

To access services

Hearing Impairment, Deaf, Cochlear Implant, Tinnitus – Eligibility

  • To be eligible for services, the person must have one of the following:
    • be diagnosed with a sensorineural or permanent conductive hearing loss
    • report persistent and significant difficulties in executing life habits as a result of the hearing loss or tinnitus
  • The referring source must include:
    • a copy of the most recent audiology report (audiogram)
    • any pertinent medical and professional reports (ENT, Occupational Therapy, Psychology, Social Work, etc.)
    • Request for Services form or the Demande de services interétablissements (DSIE) for professionals working within the Health and Social Services Network
    • all documents must be sent to the ID-ASD-PD Access Desk

Vision Impairment – Eligibility

  • To be eligible for services, the person must have one of the following:
    • a visual pathology or diagnosis
    • report persistent and significant difficulties in executing life habits as a result of a vision loss
  • The referring source must include:
    • a copy of a recent eye report (<1 year)
    • any pertinent medical and professional reports (Occupational Therapy, Physiotherapy, Psychology, Optometry, Social Work, etc.)
    • Request for Services form or the Demande de services interétablissements (DSIE) for professionals working within the Health and Social Services Network
    • all documents must be sent to the ID-ASD-PD Access Desk

Dual Sensorial Impairment and Deafblind – Eligibility

  • Referral using the Request for Services form can be made by the individual, the family or a member of the community
