Hospital@Home program has been awarded the coveted VBHC Prize 2024 at a ceremony in the Netherlands.
With its approach to hospital-level care at home, integrating remote monitoring and interdisciplinary teams, Hospital@Home has led to improved patient outcomes, roughly 4000 bed days saved, a $1.5 million return on investment and requires nearly 50% less staff.
The concept of value-based health care is directly aligned with our philosophy of Care Everywhere, which emphasizes the delivery of the right care at the right time in whichever location is most appropriate, safest and most convenient for the recipients of health care and social services.
