Services offered

  • Assessment of ocular health and functional vision through specialized optometric evaluation and equipment
  • Recommendations related to the optimal use of remaining vision
  • Recommendations pertaining to visual aids and devices that can enhance visual functioning
  • Prescriptions for contacts lenses, eyeglasses and telescopic or microscopic systems

Services are provided by professionals from the following disciplines:

  • Occupational Therapy
  • Optician
  • Optometry
  • Low Vision and Vision Rehabilitation Therapy

Youth and Adult Low Vision Clinic

  • At the MAB site
  • At one of our two satellite offices:
    • Jewish Rehabilitation Hospital, Laval
    • CLSC du Lac-Saint-Louis, Pointe-Claire

To access services

  • To be eligible for services, the person must have:
    • a specific diagnosis related to vision impairment
    • report persistent and significant difficulties in executing life habits as a result of the vision impairment
  • The referring source must include:
    • a copy of a recent eye report (<1 year)
    • any pertinent medical and professional reports (Occupational Therapy, Physiotherapy, Psychology, Optometry, Social Work, etc.)
    • Request for Services form or the Demande de services interétablissements (DSIE) for professionals working within the Health and Social Services Network
    • all documents must be sent to the ID-ASD-PD Access Desk

To make a request for services

  • Referral using the Request for Services form can be made by the individual, the family or a member of the community
  • See the Request for Service form from the ID-ASD-PD Access Desk
